Your treatment plan will include a breakdown of all applicable fees and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. Please ask a member of our staff and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our treatment coordinator prior to receiving service.

If you have dental insurance, a pre-determination of benefits (an estimate) will be submitted on your behalf. Once treatment has started, we will file your insurance claims on your behalf.

To help us with your insurance filing, please complete the insurance questions on our questionnaire. Please remember that insurance is a benefit for you or your child. If during the course of treatment your benefits change, the financially responsible party is accountable for all charges. It is important to inform our administrative staff if any changes occur to your insurance in order for your file to be updated with our office.
